Safe Not Sorry! Awards

During a post data collection de-brief for the Cognitive Workload study, a few members of the EVA and Environmental Physiology Lab (EEPL) went through an inspection of the Infinadeck multidirectional treadmill and overhead subject positioning system. This inspection of the positioning system identified multiple failed or cracked welds. Though these failed welds did not create an immediate risk to the research subjects, there were noticeable concerns around the equipment itself. Any bad welds in the subject load path were confirmed secure with a mechanical fastener (bolts that were intact).

Immediately following the inspection, the test team contacted their EEPL leadership, the NASA TM, the study PI, and the HHPC SK TRR Chairperson / Research Safety. As a result of these discussions the Infinadeck System was tagged out and the team began conversations with the hardware manufacturer. Post incident inspections identified additional structural issues with the overhead system which are also being discussed with the vendor.

The research team is recognized for their immediate response to this safety concern, identification of hardware concerns, immediate reporting up the chain of command, pausing an ongoing research study to address these matters, and leading the conversations with the vendor on a corrective action plan.

The B21 network update on 5/21/25, which interfaces with the B21 Enterprise Building Integrator (EBI) for facility maintenance and alarms, including critical systems like fume hoods, created an unknown hazard. On 5/22/25, it was discovered that no B21 systems were visible in EBI, thus no monitoring or alarming could occur, and one Environmental Chemistry Lab fume hood was reset to off. During this time the Environmental Chemistry Laboratory (ECL) personnel were calibrating combustion product monitors utilizing highly corrosive acid gases. As this work proceeded it was observed by Hadley Ransom that the hood was very quiet and suspected that it was not working properly and discontinued work immediately, took steps to safe the area, and notified management. Since then, a corrective action has been opened and revisions to work processes are being implemented to protect laboratory workers in the future.

Hadley is recognized for his attention to detail, diligence, and immediate response to this safety concern including an immediate stop work, reporting out, and actions that lead to the opening of a corrective action.
